Good afternoon. Welcome to the USJF conference at San Francisco State University. My name is Takenari Asanuma. I am a Judo seifuku therapist from Japan. I am training and working with Dr. David Matsumoto now.
I'd like to talk about "Judo Risk Reduction and Injury Management: The Judo Seifuku Therapy Perspetive". Before I talk to you about topics, I will introduce Judo seifuku therapy first.
What is Judo Seifuku Therapy?
It is a Japanese traditional medical treatment handed down a long time ago.
It is descended from Kappo of Jujitsu, which has the same origin as Judo
Judo seifuku therapy traces its roots to the records of bone and joint injuries in the 8th Century. In the 16th Century, during the Japanese civil wars, various sections of martial arts, including Jujitsu were formed.
Jujitsu has many sections, Takenouchi ryu, Yoshin ryu, Tenjin shinyou ryu.
Members of each sect were taught the original secret of Sappo, which was throwing down and killing an opponent. In contrast, Kappo was the way to revive an opponent who was injured or nearly dead.
Sappo and Kappo used to be learned and taught together. Jujitsu masters had to acquire both of them. However, in 1882, Judo was established by Dr.Kano based on the Sappo of Jujitsu. As you know, Judo has spread all over the World today. On the other hand, Kappo was relegated to Judo Seifuku Therapy, the hand technique used for caring sprains, fractures and dislocations.
Treats fractures, dislocations, bruises and sprains through manual technique.
Bloodless (non invasive)@and conservation treatment.
Judo seifuku therapy is not only a reflection of a traditional way of treating injury, but also includes modern ways of treatment, such as physiotherapy. Judo seifuku therapist use external pressure like acupressure to the body and injury under the skin. And Judo seifuku therapists treat physical injuries from fatigue. The process of treatment is first to check the injured area, then seifuku(reduction ) if necessary, fixing if necessary, and aftercare like massage by hand technique, exercise, physical therapy using various machines like ultra sound, micro current stimulator, infrared radiation, laser, hot bath etc. And then, we use the natural healing power effectively. Namely, our treatments are based on human science and natural science.
Current status in Japan
In 1920 Judo seifuku therapy was approved by Japanese government officially. At present, the national health insurance system approves treatment only by medical doctor, dentist and Judo seifuku therapist in Japan. Therefore, Japanese government requires us to have high knowledge and a national qualification which is issued by Ministry of Health Labor and Welfare after three years study at an accredited school.
The qualification requires 14 credits in basic fields, 38 credits in special basic fields like anatomy, physiology, pathology, hygiene, diagnostics, etc and 33 credits in special fields like Judo seifuku theory, critical therapeutic methods, bandaging methods. It also requires that the students must practice Judo at school. English class is not required. That's why my English is so poor. Sorry.
Topics of lecture
First topic is "Minimizing injury at Dojo
I will show a few important things on how to minimize injury at Dojo.
Second topic is "When injury occurs at Dojo..."
I will talk about immediate care prior to taking the injured people to hospital or clinic.
Last topic is "Demonstration for Jusei Kappo and Kinesio taping
technique"
Finally, I will demonstrate some jusei kappo technique- how to revive people who became unconscious (go out) by choking techniques and show some taping technique using kinesio tape.
After that, we will try to do it together. Not choke each other. Taping.
Ok, let's start first topic, Minimizing injury at Dojo
Check self condition
I will not talk about Health now.
But what I would like to talk about is the importance of safety, not only your safety but also of your opponent's. You want to make sure that you do not get hurt as well as not to hurt others.
This isn't a hard thing to do. You just need to remember to take some precautions. These are things such as taking off accessories like necklaces and earrings, keeping your nails short, if you have a wound, put bandage to protect it, if you have a cold,, take a break from practice not just for you to get well but also not to spread the cold to your teammates.
Your consideration of others will prevent you and your teammates from injures and sickness.
This mentality is a part of what Dr. Kano presented us as "Jitakyoei" co-prosperity.
Cleanliness
You always want to practice thorough hygienic management.
With strenuous practices, the tatami or mats get dirty with blood, sweat, fallen hair...
I have had the opportunities to visit many places and I got the impression that it is seldom that they sweep the tatami or mats before and after the practice or wipe the mats regularly using antiseptic solution.
These precautions prevent injuries through suffering a slip as well as from infectious disease. Just to give you an idea of what I am talking about, I would like to share one of my experiences from my high school days. I had pimples, the sign of youth,
we call it. The pimple by my mouth got infected with the germ from tatami and my whole my face got swollen like a balloon. As you could probably guess, I couldn't practice at all. When you practice newaza, you may rub your face or even be forced to kiss the tatami that is full of germ. Even if you pay extra attention in keeping the tatami clean by sweeping and wiping before and after each practice and use antiseptic solution twice a week, you may still get infectious disease like I did. You should always wear clean Gi. This is not just for sanitary reasons but also to be considerate of your practice partner. You don't want to knock the person out with your body odor but with your waza, the judo technique, right?
By the way, can you imagine what happened to the pimple after it made my face swollen? When I yelled "Ippon" in the admiration for the superb technique of uchimata of the player when I was judging a practice match, the pimple burst out. You don't want to know what I saw all over the tatami when it happened. Needless to say, the pimple was gone after that.
Practice "UKEMI"
As you know ukemi is the most effective guard for your body. I think every
Judoka have to be an ukemi expert. Ukemi is one of technique same as
uchimata, seoinage so on. So a Judo instructor has to get to teach ukemi not only to beginners but also to advance players.
Ukemi is also very useful not only when you practice Judo but also when you have an accident like falling down or slipping on the ground.
Adjust to the student's ability
It is important that instructors figure out what each student's characteristics are, such as their physical attributes, personality, suitability and determine which waza to teach and give an appropriate instruction on practice method.
Instructors need to pay extra attention when giving work that's beyond the student's capacity even if it is to increase the level. Again, it requires instructor's ability to evaluate their students' capacity as well as the ability to determine what's appropriate for the students depending on the level of the students. Also for beginners, it is ideal that instructors take time to give sufficient explanation on both safety and the risk in practicing Judo.v It is one of very important instructors' missions to provide students an environment where they can enjoy practicing Judo for a long time rather than being forced to suffer hard practices. And I believe this will help the growth of Judo population contributing to the overall development of Judo.
When Injury occurs at Dojo
How to distinguish serious injury from other injuries
In any case someone gets hurt, it is important that instructors give every possible and
appropriate first aid to the injured by the time the injured are taken to a hospital.
However, you need to remember that depending on the parts of body that injured or the condition of the injured, you should not move but keep the injured immobile. For example, if someone hurt his head, neck or spine, do not move him but check whether he is conscious or not and call 911 and provide the information of his condition. While you wait for them, make sure do not move at all. If someone has a sprain, dislocation or fracture, please make sure not to move the part but stabilize, support and immobilize. Keep it cool by applying ice and such and wait for the rescue to come or take the injured to a hospital right away.
If the injured is bleeding, you need to stop it first. Apply firm pressure to the cut part with something clean and elevate the part to a higher position than the heart level.
In case someone gets sick all of sudden, it is important to find out what caused it, whether if he has fever, is dehydrated, developed anemia and so on and also to lie down to rest in a cool area.
And then take him to a hospital soon.
There are times children express pain and suffer regardless of the seriousness of the injury and it turns out to be no problem when you check after the shock is gone and the child calms down.
How to treat the injury (R.I.C.E)
Now let me tell you what you can do before the rescue arrives or the injured is taken to a hospital. Some of you might have heard it before but the key word is R.I.C.E.
R stands for rest, I for icing, C for compression and E for elevation. This is very simple but tells you the proper treatments and I hope you remember it and use it when necessary.
Always prepare First Aid kit
Even though you are really careful, you can not engage in Judo without getting injured. You must always prepare a first-aid box and ice to cool down so that you can cope with an injury whenever it happens.
In the first-aid box, you need to have a disinfectant, band aid, sterilized gauze, cotton, tape,
bandage, and nail clipper.
In addition, it is desirable that every instructor get certified First Aid and CPR by the Red Cross or somewhere at least once to gain the knowledge for unforeseen circumstances.
Demonstration of Jusei Kappo and Kinesio taping technique
Now, I will show some revival techniques and introduce one special taping.
Ok, first I would like to introduce Jusei Kappo briefly.
As I told you, Kappo is a manner to show the respect and give treatment for the player you threw down.
For example, if somebody goes out by choking technique, you bring them back to consciousness by Jusei Kappo technique. In other words, we apply the technique of resuscitation. In Japanese, we call it as "Katsu wo ireru".
Today, I am going to introduce two basic techniques "Honkatsuho" and "Tandenteikatsuho". Both of them are the basics of Kappo. They are really useful methods. So, you can apply them in your Dojo. What is happening when people go out? Chokes stop the flow of arterial blood to the brain momentarily and cause a temporary severe anemia. These two techniques quickly turn back the flow and recover the normal state. The first one, Honkatsuho. There are two ways. One way is to let the person lie on his/her back, and the other is to raise the person up.
Let's talk about the first way let person lie on back.
Let the person who went out lie on his/her back.
Place both hands on the pit of the stomach, not on the chest.
Press the hands toward the chest several times.
Ok, let's talk about 2nd way, let person raise the upper body
Raise the upper body and stand behind the person.
Sit with your knee press against his/her backbone.
Put your hands under his/her chest.
Push up in order to open the chest while fixing your knees.
Repeat it several times.
Next I show Tandenteikatsuho Raise the upper body and stand behind his/her.
Sit with your knee press against his/her backbone.
Clasp your hands on his/her pit of the stomach.
Using your knee as fulcrums, push up in order to open the chest.
Repeat several times.
Next, I would like to introduce Kinesio taping technique.
In. Japan, not only for athletes, this tape is used for treatment widely as well as white regular tape.
Not like white tape, a specific characteristic of this tape is its elasticity.
This tape protects and strengthens a damaged muscle.
Not like other sports, judo sometimes requires large movement of a joint even though athletes are injured.
For example, an ankle requires large movement. However, white tape restricts the joint movement because of its excellent fixation.
This tape can fix a joint without restricting the movement. Moreover, this tape works to regulate the flow of lymph fluid and the stimulation of a nerve.
Of course it is more effective to use together with white tape.
Ok, I show tape technique for ankle today. Someone volunteer who has problem with ankle.
Well, now I want you to use this tape by yourself so that you can actually know how it works.
This is an effective technique for an injury, such as calf muscle rupture and achillodynia (pain of Achilles)
First, you will lay the injured person down on his/her stomach.
Loosen the muscle that is damaged, and then put this tape from the back of the foot to the back of the knee along an Achilles tendon. You have to do this as you pull this tape. Pull the tape taunt, no wrinkles.
If the injury is serious, you can do the same thing twice or three times. The tape will be on top of each other. Or, you can also use this tape around an ankle to support and limit movement.
The good thing about this tape is that the elasticity of this tape would protect the damaged muscle and the muscle won't stretch beyond its range and yet remains flexible.
